Highland Community Link Project
A Community Link Worker (CLW) is an employee of Support in Mind Scotland who works within the GP practice to provide a person-centered service to registered patients.
The Community Link Worker offers support to patients, enabling them to set goals and overcome barriers, in order that they can take greater control of their health and well-being. Using 'good conversations' a CLW supports patients to identify problems or issues they are experiencing and to talk about what really matters to them. They supprot patients to achieve their goals by enabling them to identify abd access relevent resources in their community.
Anyone over 18 can be referred by their GP or Nurse to this service. They offer appointments face-to-face, Using Near me, video-conferencing or by phone.

“All information patients share with the Community Link Worker Team is treated confidentially. However, if there is a risk of harm to an individual, or of harm to others, then the CLW team have a duty to share that information with an appropriate referral organisation, such as the GP Practice, Emergency Services, or Mental Health services.”
