When you register with the practice you will be asked to fill out a registration form and health questionnare.  You will be asked to make an appointment for a new patient medical with a doctor/practice nurse to obtain a brief history of previous illnesses and routine tests will be carried out.

Preference of Doctor and How You Can Access This

Patients are registered with the practice, not an individual GP.  For administrative reasons your medical card will be issued in the name of one of the doctors; however you can request to see a particular doctor if you so wish and we will do our best to respect your choice.

Please remember to complete both the New Patient Registration form AND the New Patient Questionnaire form or your forms will be returned to you.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 1 days. After that you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

We will need you to complete a Temporary Resident form which can be found below or you can pick one up from reception.

All three of our surgeries have suitable access for disabled patients, and all the patient areas including waiting room, consulting rooms and toilet have wheelchair access.

A wheelchair is available at reception.  Designated disabled parking spaces are located nearest to the entrance of the Medical Centre.
